Items to Bring with You for Your Interview

  • Download and Print Out the Background Consent Form (ALL APPLICANTS)
  • Fill out the online I-9 form and print it when completed
  • Fill out the online W-4 form and print it when completed
  • BRING these three (3) documents, PLUS the correct State Tax Withholding Form (provided below) along with two (2) forms of ID*, a printed copy of your resume, a voided check, and your list of supervisory references.
    *If you are not sure about which forms of ID to bring, please contact your consultant.
